Van Request Form
Crossroads Nazarene Church offers a church van for ministry-related transportation needs. Our van comfortably seats multiple passengers and is available to ministry leaders and authorized personnel for church-approved activities. To ensure responsible and efficient use, all drivers must meet the church’s vehicle policy requirements, and the van should be returned with a full tank of gas. Please complete the van usage form to sign out the vehicle, including details of your trip, the expected number of passengers, and the purpose of the trip.
Expense Reimbursement Form
For those who incur expenses on behalf of Crossroads Nazarene Church, we offer an easy and straightforward reimbursement process. This form is for ministry leaders, volunteers, and staff to submit receipts and documentation for church-related purchases. Please include the reason for the expense, itemized receipts, and any applicable approvals from ministry leadership. Reimbursements will be processed according to the church's financial policies and paid promptly.

Volunteer Clearance Packet Information

As part of our ongoing commitment to creating a safe and secure environment for everyone, especially the children and vulnerable individuals in our care, we require all volunteers to complete certain clearances before they can begin volunteering. These clearances are not only a legal requirement but also a critical component in maintaining the trust and safety of our community.
Clearance Requirements:
PA State Police Criminal Record Check
PA Child Abuse History Clearance
FBI Fingerprinting Clearance (Required if you have not resided in Pennsylvania for the past 10 years)
Signed Affidavit (If you have resided in Pennsylvania for the past 10 years)
Ministry Safe (Please send your email to alonna@crsroads.org and she will send you the information)
Important Notes:
If you have not resided in Pennsylvania for the past 10 years, you are required by law to complete the FBI Fingerprinting Clearance.
If you have resided in Pennsylvania for the past 10 years, you must sign the attached affidavit, which certifies your residency and waives the need for FBI clearance.
Please complete all necessary steps and return the required documents to us as soon as possible. Your timely cooperation in this matter is essential, as we cannot permit you to begin volunteering until all clearances are received and approved.

How to Obtain Your PA Child Abuse ClearanceOpen your web browser and visit the Pennsylvania Child Welfare Portal: https://www.compass.state.pa.us/cwis/public/home.
If you do not have an account, click "Create Individual Account" to start the registration process. (Remember the Keystone ID that you create)
If you already have an account, select "Individual Login" and proceed to log in with your credentials.
After submitting, you'll receive an email with a temporary password. Use it to log in and set a new password.
Complete the Clearance Application
Once logged in, click on "Create Clearance Application."
Please follow the prompts to fill out the information you need, such as your personal details, addresses, and information about your household members.
Please carefully review your application before you submit it.
At the end it will ask the reason for your application. Select “Volunteer” and type in Crossroads Church of the Nazarene” as the organization.
To get results, simply log out and log back in. You will see a green highlighted area on the bottom of the screen that says your application has been processed. Print out your certificate. If you do not see your completed clearance, give a few hours even a few days and try it again.
Please print a copy of the clearance and hand it to the office secretary.
